We're looking to go to an online format for all of our registration and medical forms, so I need either a software to both make the forms in and have them submitted (kind of an all-in-one solution), or just host the pdf forms where the parent can go in and fill out the form and then submit it, but it has to follow HIPAA guidelines. I hope that makes sense!
Hi Mandy. Your question makes perfect sense, and is a good one to ask.I did a quick search and found a few online form companies (including formstack, practisforms, and 123formbuilder.com) that say they offer HIPAA-compliant forms.
I also found these articles. Among other things, they advise you to require a Business Associate Agreement from the form hosting company.
